| varenicline 0.5 mg orally, daily initially, titrated up to 1 mg 12-hourly. The dose should be titrated slowly as follows: day 1 to 3: 0.5 mg daily day 4 to 7: 0.5 mg twice daily day 8 and ongoing: 1 mg twice daily until the end of the 12 week course. For people with a creatinine clearance of less than 30 mL/min, start with 0.5 mg daily for 3 days, and then increase to 1 mg daily.
